OTEIM Lead Inquiry Privacy Policy

This policy explains how OTEIM handles information submitted through its Facebook and Instagram program inquiry forms. It applies to these lead forms and related inquiry follow-up. Clinical records, payment information and any services you later purchase are handled through their separate intake and service processes.

Information you provide

We collect the contact details requested in the form, such as your name and email address, together with your program or location preference, availability and answers about the program information shown. Meta also provides the form submission details and the ad or campaign associated with your inquiry. Please provide an adult's contact information. These forms are for adults aged 18 or older and do not ask for a child's name, diagnosis, medical records or other health information.

How we use your inquiry

We use this information to respond to your request, discuss the selected program, check availability, arrange a discovery call and keep track of follow-up. Submitting the form asks OTEIM to contact you about your inquiry; it does not reserve a place, create a treatment relationship or obligate you to purchase. You may ask us to stop contacting you at any time. Joining a separate promotional mailing list is not required to request program information.

Who can receive the information

Your submission is provided to OTEIM through Meta. OTEIM's team and the service providers used to store inquiries, manage contacts, communicate and schedule calls may process it for those purposes. These tools may include Meta, Google Workspace, Wix, email services and Calendly. We may also disclose information where required by applicable law or to protect legal rights. OTEIM will not sell or rent contact details collected through these inquiry forms or upload medical information from an inquiry to advertising audiences.

Facebook and Instagram process information under Meta's own privacy policy, available at https://www.facebook.com/privacy/policy/. If you choose to visit our website or book through Calendly, those services may also collect technical information and cookies under their applicable notices and settings. This lead-form policy does not describe or change those platforms' independent data practices or browser tracking settings.

Storage and international inquiries

OTEIM is based in California, United States. Your inquiry may be accessed or processed in the United States and other countries where the service providers operate. We keep inquiry information for follow-up and related administration while it is needed for those purposes, or longer where applicable obligations require it. Contact us to request deletion or to ask about the information held for your inquiry. Do not send sensitive medical or payment information through an advertising form or social-media message.

Your choices and requests

Please identify the name and email used on the form so we can locate the submission. We may need to verify your identity before responding. Requests are handled subject to applicable requirements and any information we are required to retain. You may have additional privacy rights under the laws where you live.

Changes and contact
